Piriformis Syndrome
The piriformis is a small muscle deep in the buttock that helps rotate the hip. When it is tight or irritated, it can ache on its own and may press on the sciatic nerve that runs close beside it.

People often describe a deep ache in one buttock that gets worse with sitting, driving, or climbing stairs, sometimes with pain that travels down the back of the leg.
About piriformis syndrome
The sciatic nerve passes right next to the piriformis, and in some people it runs through or under it. When the muscle tightens, swells, or goes into spasm, it may irritate the nerve and cause symptoms that feel like sciatica.
It can be hard to tell apart from sciatica that starts in the spine, such as from a disc. That is why an evaluation by a healthcare provider matters, especially if you are not sure where your pain is coming from.
Common symptoms
- A deep ache in the buttock
- Pain that may travel down the back of the thigh
- Pain that is worse with prolonged sitting
- Discomfort climbing stairs or rising from a chair
- Tenderness over the buttock
- Tingling or numbness in the buttock or leg
Common causes
- Overuse from running, cycling, or long periods of sitting
- A direct fall or bruise to the buttock
- Sudden increase in activity
- Tight or weak muscles around the hip
- Differences in how the sciatic nerve sits near the muscle
Piriformis syndrome and sciatica
Sciatica describes where the pain goes. Piriformis syndrome is one possible reason for it. Others include a disc pressing on a nerve root and a narrowing of the spinal canal. Which one applies to you changes what helps, and your healthcare provider is the person to sort that out.
How High-Pressure Cryotherapy (HPC) may help
High-Pressure Cryotherapy (HPC) uses high-pressure carbon dioxide (CO₂) to rapidly cool the skin over a specific treatment area. For piriformis syndrome, treatment goes on the sore area of the buttock. HPC does not release a muscle from the inside or change how the nerve sits, and it does not claim to. For many people it may help manage pain and inflammation in the area, though results vary. If your leg symptoms are significant, we want your healthcare provider involved. A typical series is 4 to 5 sessions, then we reassess. Many patients report relief after one or two visits, although individual responses may vary. If there is no meaningful improvement after the series, see a qualified healthcare provider. HPC does not release the muscle or move the nerve. It treats the sore area, and for many people it may help manage pain and inflammation.
Every treatment begins with a free consultation. Typical treatment takes about 3–6 minutes per area. HPC is not a substitute for medical care and does not replace a diagnosis.

Frequently asked questions
Is piriformis syndrome the same as sciatica?
Not exactly. Sciatica is the symptom of pain along the sciatic nerve. Piriformis syndrome is one possible cause of it, and there are others. A healthcare provider can help tell them apart.
Can HPC help piriformis pain?
HPC treats the sore area, and for many people it may help manage pain and inflammation. It does not stretch or release the muscle from the inside. Results vary.
Should I check with my doctor first?
If you have not had your buttock or leg pain evaluated, or you have leg weakness, numbness, or changes in bladder or bowel control, please see your healthcare provider first. We review your history at a free consultation.
How soon can I be seen?
Same-day appointments are often available at our Mineola and Rockville Centre locations. Book online or call and we will find the earliest time that works for you.
How many visits might I need?
For most people, we recommend an initial series of four to five sessions, then we check how you are doing. Many patients report relief after one or two visits, although individual responses may vary. Your plan depends on your situation, and we review how you are doing along the way. Results vary.
What does a visit cost?
The first treatment area is $90, with lower prices for additional areas, and discounted packages are available at our Rockville Centre location. See our pricing page for details.
